This report only considers the districts which have shifted to facility wise data uploading on the HMIS portal; This report excludes the performance of medical colleges; # : Total No. of Facilities refers to all the public facilities which are active and physcially existing; * : No. of facilities reporting Nil Performance includes (a) Facilities which haven't reported for the stated period (b) Facilities who have reported for the stated period but haven't reported for a specific data item and (c) Facilities that have reported zero ('0') performance; ** : Overall Average=Sum of value reported for a particular indicator in a particular period by facilities (SC/PHC/CHC/SDH/DH) / Total number of facilities that have not reported Districts reporting facility wise means Districts reporting at facility level for all the months of the reporting period May, 2018; *** : There may be differences in number of facilities with the total number of facilities (in Column C) to the number of facilities reporting nill performance (in Coulmn D) and Number of facilities by performance due to the upgradation/degradation and deactivation of the facilities in the HMIS Master; Data updated As on 22 Nov 2019, 2:07 PM.
